WPBF 25 News Investigates has exclusively obtained the autopsy report in the death of Madison Warner, the eldest daughter of Virginia Sen. Mark Warner, revealing new details about her final hours and an unusual medical conclusion.
According to the Palm Beach County Medical Examiner’s report, 36-year-old Madison Warner was found unresponsive in her bed at Good Samaritan Medical Center on April 19 after being hospitalized for two days.
The nursing supervisor noted she had been admitted due to complications from anemia.
The report states that at approximately 5 p.m., a visitor left Warner’s room. Ten to 15 minutes later, nurses found her unresponsive with a syringe in her hand.
The report does not say what was in the syringe.
Hospital staff attempted lifesaving measures for about 30 minutes before Warner was pronounced dead at 6 p.m.
According to the report, the medical examiner investigator who responded to the hospital advised staff to contact police because of the circumstances surrounding the death. West Palm Beach police opened a criminal investigation.
A source confirms they were able to locate the syringe in a waste receptacle and sent it for testing, which has not concluded.

The medical examiner concluded Warner died from “complications of acute on chronic IV injection of oral medications.”
The terminology indicates that medications intended to be swallowed had been injected into the bloodstream, causing both recent and long-term damage.
However, the medical examiner ruled Warner’s manner of death “undetermined.”
Deaths are generally classified as natural, accidental, homicide or suicide.
What is not clear is how to classify Madison’s death.

The pathologist wrote that Warner’s lungs contained “innumerable impregnated polarized crystals surrounded by extensive lung tissue scarring.”
The medical examiner did not attribute Warner’s death to drug toxicity or overdose.
Toxicology testing found several medications in her system, including alprazolam, pregabalin, antipsychotics and other prescription drugs.
Madison’s medical history was listed to include:
Type 1 diabetes, hypertension, chronic kidney disease, Von Willebrand’s disease and ulcerative colitis.
The report also notes that repeated attempts to obtain Warner’s prior medical records from her home state were unsuccessful.
“The absence of this medical information precludes the PBCMEO’s duty to definitively complete our medicolegal death investigation for this case. Thus, the manner of death cannot be determined,” the report states.
The medical examiner noted that if additional information is provided, the cause or manner of death could change and an amended death certificate and autopsy report could be issued.
Sen. Mark Warner previously announced that his daughter died from complications of juvenile diabetes.
After being contacted by Investigative Reporter Terri Parker, a spokeswoman for the Warner family issued a statement:
“Madison faced significant physical and mental health challenges throughout her life, and those who knew and loved her understand the strength and resilience with which she met them. To her family, Madison was first and foremost a beloved daughter whose absence is felt every day.
“Although her father’s service has drawn attention to this tragedy, Madison was not a public figure, and the family respectfully asks for privacy, compassion and understanding as they continue to grieve this tremendous loss,” the statement said.
The family did not address the specific findings contained in the autopsy report.
West Palm Beach police confirm after extensive investigation, they have closed the case.
